Separate the evidence from the decision
A document is an input to a review. A clearance decision records the outcome for a particular placement. Keeping the two separate helps explain what was reviewed, who reviewed it and why that evidence was considered relevant to the assignment.
Give each requirement an owner
List the information needed for a placement and assign responsibility for collecting and reviewing it. Use explicit statuses for missing evidence and pending review. A file being present should not be treated as proof that somebody has assessed it.
Plan for change and expiry
Review the requirements when an assignment changes or is extended. Some evidence may remain relevant; some may need refreshing. Keep the current status and the historical review trail distinguishable so an old decision is not mistaken for a current one.
